- Permission to Administer Medication FormView Form
Note: For all medications, emergency or routine, to be administered to a student, both parent permission and physician’s orders must be on file in the health office. Care plans, emergency plans, or actions plans for all medical conditions must be updated every school year. All medications that are required by a student must be provided to the school in the original package, labeled with student’s name. Medications must only be delivered to the health office personnel, not sent into school in a student’s bag. A receipt of medication delivery will be provided to the parents when medication is received.
