The following forms will need to be completed as soon as possible but no later than Monday, August 25, unless otherwise indicated. 

  • The Back to School Form for all students.
  • Busing and Dismissal Form on FACTS Family Portal.

Health Forms:

  • NYS School Health Form (or Child in Care Medical Statement) and immunization records to be completed by your child’s physician. WHMS requires a current physical and immunization records for all students.
  • Authorization to Administer Internal Medication by School Personnel form, if applicable.
  • Sunscreen, insect repellent, and tick removal consent form, if applicable.

Register your child in your local school district. All students from K-8 must be registered at their home district for busing and textbooks.
