The following forms will need to be completed as soon as possible but no later than Friday, August 21, unless otherwise indicated. 

  • Back-to-School Form

  • Busing and Dismissal Form

  • Health Forms:

    • NYS School Health Form (or Child in Care Medical Statement) and immunization records to be completed by your child’s physician — WHMS requires current physical and immunization records for all students.

    • Allergy Action Plan, if applicable

    • Authorization to Administer Internal Medication by School Personnel Form, if applicable

    • Sunscreen, Insect Repellent, and Tick Removal Consent Form

Register your child in your local school district. All students from K-8 must be registered at their home district for busing and textbooks.
